REQUIRED:
ORIGINAL NEGATIVE COGGINS DRAWN WITHIN 6 MONTHS of Closing date of the auction. Fax copy of Coggins to 877-202-2552.

SEND TO P.A.S.INC. - With mailed entry or within 5 days of Faxed Online entry. Completed Entry form, ORIGINAL Registration Certificate. Signed transfer, Copy of Coggins, HYPP report, if applicable, and Pregnancy Exam if applicable.

FAXED ENTRIES:
• Fax COMPLETED Consignment Form, Disclosure Form, Horse Information Form, Copy of Registration Certificate & HYPP Results (if applicable).
